Please note that the following information is included on this schedule:
- The city where the clinic is located;
- The address of the clinic;
- The name of the low vision doctor, his/her phone number, and fax number; and
- The name and number of the counselor from the Department of Labor/Rehabilitation (VRC) in that specific city; (the counselor should be contacted if you are visually impaired and working, looking for a job, or the primary homemaker).
